Please bring the following to your first appointment:
- Immunization Record
- Medical records from your previous physician.
If you do not have records, please complete this Medical-Release-Form and send it to your previous physician so that your records can be forwarded directly to our office. If you have difficulty downloading and printing the form, you can drop by our clinic and we will be happy to provide you with this form.
Please note that we at Sang Pediatrics feel strongly that vaccines protect children from many severe illnesses.

We are a pediatric primary care provider for the Fresno and Clovis area. We provide multiple pediatric services for your child's healthcare. We are also available for walk-ins during our regular business hours at any of our locations.
